Membership Application |
Used to apply for and renew annual membership. Renewal is due by February 1st of each year. |

2024 Stallion Nomination Form |
Used to nominate your stallions so that you can nominate their foals so that the foals can earn show points to win incentive funds. This form is for the 2024 breeding / 2025 foaling year.

Stallion Intent to Travel or Show Form |
Used to inform the Kentucky Horse Racing Commission (KHRC) that your stallion will not be on your farm in case they send an inspector to check for stallion location. |

2024 Mare Nomination Form |
Used to nominate your mares so that you can nominate their foals so that the foals can earn show points to win incentive funds. For the 2024
foaling year, must be submitted by Feb. 1, 2024 |

Foaling Verification Form |
Used by out of state mare owners to have veterinarian certify that the foal is born in Kentucky. This form must be signed by a licensed veterinarian within 48 hours of foal’s birth and received by KMHBC within 30 days of verification of the attending veterinary. |

Embryo Transfer Form |
Used for Veterinarian's report of embryo transfer. |

Youth Nomination Form |
Used to nominate a youth to participate in the KMHBC youth incentive fund. If a youth/horse combination has already been nonminated, you do not need to re-nominate the youth/horse combination each year.
