Following are the forms (click to download) you will be using to participate in the Breeders Incentive Fund:
| 2010 Stallion Nomination Form |
Used to nominate your stallions so that you can nominate their foals so that the foals can earn show points to win incentive funds. This form is for the 2010 breeding / 2011 foaling year.

| Stallion Intent to Show Form |
Used to inform the Kentucky Horse Racing Commission (KHRC) that your stallion will not be on your farm in case they send an inspector to check for stallion location. |
| 2010 Mare Nomination Form |
Used to nominate your mares so that you can nominate their foals so that the foals can earn show points to win incentive funds. For the 2010 foaling year, must be submitted 30 days prior to foaling. |
| 2010 Foal Nomination form |
Used to nominate your 2010 foals so they can earn show points to win incentive funds.

| Foaling Verification Form |
Used by out of state mare owners to have veterinarian certify that the foal is born in Kentucky. This form must be signed by a licensed veterinarian within 48 hours of foal’s birth and received by KMHBC within 30 days of verification of the attending veterinary. |
| Embryo Transfer Form |
Used for Veterinarian's report of embryo transfer. |
